A free SecondYou pilot.

This is a free SecondYou pilot—not a sales call. Selected families pay nothing during the trial, with five additional years of service included afterward under the pilot terms, subject to continued commercial availability.

SecondYou is an AI phone companion built from the family caregiver’s voice. You listen to and approve that voice before your loved one uses it.

Our recruiting team can explain the trial and help with the application. There is no obligation to apply.

We’re inviting 25 families.

Participation is free for selected pilot families. For enrolled families, the service is free during the pilot and for five years after the pilot ends, as long as it remains commercially available.

Offer and participation details

For this offer, enrolled means accepted into the pilot, informed consent completed, and onboarding begun. Later pausing or withdrawing does not by itself end that eligibility or create a charge or repayment obligation.

Participation includes informed consent, guided setup, voice review, initial calls together and feedback. During the current pilot, a trained human from the team is live on every call. SecondYou is not an emergency or safety monitoring service.

Experimental pilot and selection

Enrollment in this pilot is offered to caregivers of people with dementia. The caregiver, not the person receiving calls, is the applicant and is responsible for screening, consent, setup, and feedback. This is an experimental product. Some features may be unavailable, may change, or may not function as intended during the pilot. No results are guaranteed. Individuals may meet the criteria, but not be selected. Selection for the pilot program is solely at the discretion of ProxiCare PBC.

Five-year free access for pilot families

For this benefit, enrolled means a family has been accepted into the pilot, completed informed consent and begun onboarding. Enrolled families receive the SecondYou service at no charge during the pilot and for five years after the pilot ends. Features included in that service and offered to paying families remain free to enrolled pilot families during that period. There is no automatic billing. Once a family meets this definition, later pausing or withdrawing from pilot participation does not by itself end eligibility for this five-year free-access offer, as long as the service is commercially available. Withdrawing from the pilot does not create a charge or repayment obligation.

Program and service changes

ProxiCare PBC may cancel, change, modify or end the pilot program at any time. The five-year free-access offer applies as long as the service is commercially available. ProxiCare PBC may change features or discontinue the service for safety, legal, technical, operational or business reasons. If the service is no longer commercially available, ProxiCare PBC is not required to continue operating it solely for pilot families.

General wellness only

This program is a general wellness pilot testing program and is not a clinical trial. It is intended to explore and evaluate the use of the system for general wellness purposes and is not designed to diagnose, treat, cure, or prevent any disease or medical condition.

Participation does not guarantee any particular outcome, benefit, improvement, or result. Individual experiences and results may vary. The system and program should not be considered a substitute for professional medical advice, diagnosis, or treatment.

I understand that, if I am accepted and later give informed consent, my loved one may receive phone calls from SecondYou, an AI system, using my name and a voice modeled on mine. I understand that, during those calls, my loved one may not be told that the voice is generated by AI. This will be fully explained before I decide whether to give informed consent. I understand the summary about voice recordings, transcripts, and how the AI model is handled for the pilot. I consent to be contacted by SecondYou or the recruiting organization for follow-up. I consent to future contact if not selected for the first pilot cohort.

About this application

This form is for a small pilot inviting 25 families caring for people with dementia, recruited with the Ellen Samson organization. The caregiver completes this application and participates in screening, consent, setup, and feedback. "Loved one" means the person you care for.

This is screening only. Submitting does not enroll you or guarantee acceptance. Selection is solely at the discretion of ProxiCare PBC. Nothing starts unless you are accepted and then read, ask questions about, and sign an informed consent agreement. Any voice recording happens only after that.

Your privacy. What you share stays inside the screening and pilot review process. Your name and contact details stay with the recruiting organization until you are accepted. Until then, reviewers see an application ID instead of your name, a "blinded application number." Submit once. After you submit, we show your next steps and your application number.
